Injury, vehicle accident, property damage, or a stuck van? Call Amazon LMET at (800) 942-3300 first when you can, then submit your badge number, what happened, and the photos requested. Dispatch reviews every report and may send it back if something's missing.
Request to Excuse a Call-Out
Missed a shift for a reason you'd like reviewed? Submit your badge number, the date, the reason, and a photo of any documentation. HR reviews each request.
Expense Reimbursement
Paid for something out of pocket that Ready Rabbit should cover? Enter your Paycom code, the purchase details, and a photo of your receipt. Finance reviews each request.
Driver Trainer Form
